All patients age 40 to 74 (who do not have a pre-existing health condition) will be invited for an NHS Health Check every 5 years.
We also invite patients annually with the following long-term conditions for an appointment with a doctor, nurse or clinical pharmacist
- asthma
- COPD (chronic obstructive pulmonary disease)
- dementia
- diabetes
- hypertension
- LD (learning disability)
- severe mental health conditions
